Medication Administration Cert I- Initial (1)

This is a 8 hour per day, 2 day class.  Prior to acceptace to the class you must submit the DODD application for Personnel to Attend the MA Certification Class.  See Link in this email. There is a lot of information to cover, and recommed that participant bring a packed lunch, as we will continue to move along after our break.

In order to be eligible to take this class, you must:

• Be at least 18 years of age
• Have proof of HS Diploma or GED
• Actively employed at an agency or current independent provider
• Criminal Background Check is completed, returned and without any diqualifying offenses

In order to complete the class successfully, you must:

• Attend the entire training class
• Actively participate in the training program, class discussions, and activities
• Successfully perform a return demonstration of proficiency
• Complete the closed book final, written exam, with scoring of 80% or better
• Complete and submit to the registered nurse trainer, the evaluation of the training program

Once application is approved and seat is reserved, an email will be sent with an invoice for payment via Wave.  You have the option of paying electronically, by check through mail or in person the day of class.  Payment has to be received prior to the start of class to participate.

Any cancellations after payment is received within 24 hrs of class will secure a seat in an alternate date, but no refund will be given.  No shows for any part of the class will still be charged the seat cost with restart of participation at next class at additional seat cost.  Unforseen circumstance may be approved for other arrangements, on a case by case basis.
